Before submitting your quote application please read the below data protection statement and read it to anyone over the age of 16 that you wish to be covered on this plan, or inform them of its content.
April UK (Insurance Services) Ltd will hold and use information about you and any of your family members to set up and manage your plan. This information may have been supplied by you, those family members or health care providers. If your plan is part of a group scheme this information may have been provided by your employer. If a family member is over the age of 16, please only provide us with sensitive information (such as health information) if you have their consent to do so. If April UK (Insurance Services) Ltd is provided with this information we will take this as confirmation that you have consent.
April UK (Insurance Services) Ltd uses other companies to do some of our work and to run, maintain and improve our computer systems. We communicate with your healthcare providers and we provide non sensitive information to your intermediary (if you use one). If your plan is part of a group scheme, we provide non sensitive information to the employer and the employer’s intermediary (if they use one).
We send most communications about the plan and about claims to the lead policyholder. The lead policyholder is the legal owner of the plan. If a person intended to be covered by the plan is over 18 and does not want us to do this then that person should apply for their own plan.
By submitting your application you confirm that:
- Any family members have agreed that the lead policyholder can act for them to set up this plan
- The lead policyholder consents on behalf of those family members and themselves to April UK (Insurance Services) Ltd using personal information in the ways described above
Information may be passed to third parties. These third parties will include other insurers and law enforcement agencies. We may pass information directly to them or through shared databases. This is to prevent or investigate crime, including fraudulent or other improper claims. In some circumstances we must give information about our suspicions of crime to law enforcement agencies. We must also let the relevant regulatory body know when we have good reason to question a healthcare provider’s fitness to practice.

April UK (Insurance Services) Ltd is committed to delivering the rights that the Data Protection Act 1998 and Privacy and Electronic Communications Regulations provide to individuals.
